Speech Contest Registration Form

Please use the formatted registration form to register your students. The spreadsheet three tabs: "Instructor Info", "Student Registration" and "Speech Contest Registration Categories". Please fill out the "Instructor Info" and "Student Registration", and refer to the "Speech Contest Registration Categories" to find the categories that your students belong to.

Instructor Info: This is where instructors fill out their information. This tab is NOT for students.
Student Registration: Please enter student registration info here.
Speech Contest Reg Categories: For instructors and students to check the categories.

Students and parents should fill out the "Student Registration" part of the form and send it to instructors. Instructors will collect all the student information, fill out the instructor info part, and then email it to [email protected] before March 23rd, 2012.
